FACILITY CONDITION SURVEY- AKIM MID TAFO

INSTRUCTIONS: Those assigned to conduct this survey should do so along with the users of the facility who
are responsible for maintenance. They should take the following actions: Safely examine the entire property, in-
cluding each building, to determine the current condition. Determine if an established maintenance program and
the required safety inspections are being carried out according to an established schedule. Identify any areas of
concern, especially safety hazards. Check that the brothers know how to carry out the upcoming maintenance
tasks safely and have access to the equipment they need in order to do so safely. Ensure that they are able to
complete a Job Hazard Analysis (DC-83) or Congregation Job Hazard Analysis (DC-85) properly for each po-
tentially hazardous task. Provide a clear description of any problems in the “Follow-up Points” section of this sur-
vey, indicating who is responsible for promptly correcting the problem. Discuss the results of this survey and the
follow-up points with the facility oversight, and leave a copy with them. Those assigned to conduct the survey
should also complete the Facility Life-Cycle Analysis (DC-97) workbook and send both of these completed items
to the Local Design/Construction Department (LDC).

In the case of a Kingdom Hall, the brother assigned to conduct this survey should follow the above steps with all
available elders. Therefore, he will invite all available elders to accompany him while he examines the property
and to discuss the results of the survey.

SAFETY CHECKLIST

Fire Safety
Are fire extinguishers located at main exits and all other places required by law, and are they tagged with
annual proof of service or inspection?

Are fire-hose or sprinkler systems in good working condition?

cations?
✘ Do dwelling-unit kitchens have fire detectors? Has grease and dirt been cleaned from ovens, hoods,

and with no breaches in compartmentation from building works?
✘ Has the emergency lighting been tested and found to be working with an appropriate discharge time? Are
batteries, where used for these systems, not over five years old?
✘ Are all exits free of obstructions, and are they marked with compliant signs?
✘ Do exit doors always operate easily when the building is occupied, with panic hardware (if required)?
✘ Is the storage of flammable paints or liquids eliminated or kept to a minimum, and are they stored in their
original containers? Nonflammable water-based paints and cleaning supplies and no aerosol containers
are recommended wherever possible.
✘ Are electrical closets and mechanical rooms free of all combustible materials, including gas-powered tools
or equipment? Are any areas where combustible materials are stored, including the attic, fitted with fire
detection and compartmented (where required)?
✘ Are heating flues properly isolated from combustible materials and regularly serviced by qualified person-
nel?

propriate interval (wherever legally required) and found to be safe?

Slip/Trip and Fall Prevention

Are mops and “wet floor” signs on hand for spills or water brought in by footwear or for use when bath-
rooms and other hard floor surfaces are being cleaned?
Are entry-floor mats in good condition, without curled-up edges, and fixed in a position that will keep them
from sliding around?
Are outside walking areas (such as parking lots and sidewalks) in good condition and free of holes or trip
hazards?

✘ Are the entire grounds free of holes, wires, or other hazards that could cause injury, even to uninvited visi-
tors after dark?
Are roofs, roof drains, and gutters in good repair and cleaned regularly so they function properly, with no
signs of leaks?
✘ Are basements, interior surfaces of exterior walls, ceilings, bathrooms, and other areas where moisture
could accumulate and cause damage free of any signs of moisture problems?
✘ Has an annual termite inspection been performed? (This is not necessary if there are no structural wood
components.)
✘ Have carbon monoxide and radon alarms been checked, and do they function properly (if required in the
building or dwelling unit)?
Is a first-aid kit present on-site (where required), with all supplies in place and not expired?
