    AFFIDAVIT CUM DECLARATION

    I, Kalpesh Ugarcand Gadhecha, a residing at 30, Sankarbharti Society, Ankur Road,


    Naranpura, Ahmedabad-380013. I have Chartered Accountants since 2000. I have not
    applied the “Certificate of Practice” (COP) till the date.

    I have got the Professional Tax No. PEC 0151133757 for Chartered Accountants practice
    address at SF/10, Anjalee House, C G Road, Ahmedabad – 380009 but actually I have not
    applied the COP till the date to “The Institute of Chartered Accountants of India” (ICAI).

    I request to you to cancellation of my professional tax no. PEC0151133757, for the


    reason of I have not done any business or profession.
