We, first year students of the College of Medical Laboratory Sciences of Iloilo Doctors’
College, are working on a survey entitled Frequency of Dealing and Coping with
Anxiety and Depression of Senior High School Students of Iloilo City National
High School, in partial fulfillment of our requirements in Community and Public Health
for Medical Laboratory Sciences. Our survey aims to determine the frequency of senior
high school students in dealing with anxiety and depression and how they cope with it
during the school year 2022-2023.

In line with this, we would like to ask permission from your good office to allow us to
conduct a survey with the senior high school students of your school. The data are
needed in the sampling procedure in the aforementioned survey. Rest assured that the
information of the respondents will be treated with utmost confidentiality in accordance
with Republic Act 10173 or Data Privacy Act of 2012.
