EXECUTIVE SUMMARY

As a part of a summer internship program, I was allowed to experience work with the trained
professionals of the hospital industry and learn about the ways of managing operations in the
DIALYSIS DEPARTMENT.
Throughout my training period in FORTIS HOSPITAL I have learnt floor auditing. I have
checked every patient’s file, whether all the documents were properly arranged according to the
NABH guidelines ..I have done time motion study in the dialysis department, the average time of
stay of the patient, how much time the technician took for priming, how much time the nurses took for
dressing.
Lastly, to summarize, my overall experience has been a very benefiting. It was a good learning
experience for me and gave me the first exposure to gain knowledge about the working of the hospital
industry.

REVIEW OF LITERATURE

J Biomed Inform. 2014 Jun:

First defined in industrial engineering in the early 20th century, time-oriented research refers to
a set of data collection methods in which an external observer captures the subject by obtaining
detailed information about the time and movement required to complete a specific task and
conduct research aimed at improving the subject. competence. . It has since been validated by
biomedical researchers and clarified by increasing interest in events related to medical work.
However, attempts to summarize these studies have been difficult due to significant differences
in methodology and reporting. Despite efforts to standardize data and findings, there is still no
consensus on what "time science" is. This not only affects the analysis, but can also explain
some of the structural (temporal) changes observed in the literature in this area. number of tasks,
multitasking, rigor of training, and reliability tests) are due to testing different sub-techniques.
We describe in detail the different methods used in conversation or classified as the "study of

many types of research of which education is not only an important part. . time capture and/or

it happens
 To have a clear concept of NABH guidelines
 To identify the proper time motion study of the various department
 To calculate the average length of stay
 To done the floor auditing
 Ro identify if there any drawbacks or problems occurring in the Dialysis department
 To be able to provide proper suggestions for the betterment or improvement of the
respective problems
ABOUT THE DEPARTMENT
DIALYSIS DEPARTMENT

In medicine, dialysis is the process of removing excess water, solutes, and toxins from the blood
in those whose native kidneys have lost the ability to perform these functions in a natural way.
This is referred to as renal replacement therapy.

Dialysis may be used in those with rapidly developing loss of kidney function, called acute
kidney injury (previously called acute renal failure); or slowly worsening kidney function,
called Stage 5 chronic kidney disease (previously called chronic kidney failure and end-stage
renal disease and end-stage kidney disease).

Dialysis is used as a temporary measure in either acute kidney injury or in those awaiting kidney
transplant and as a permanent measure in those for whom a transplant is not indicated or not
possible.

DIFFERENT TYPES OF DIALYSIS :

DIALYSIS

HEMODIALYSIS PERITONEAL DIALYSIS

HAEMODIALYSIS:

Hemodialysis, CRRT and SLED are similar procedures and use the same principle (membrane
filtration). At the beginning of the session, a needle is inserted into the AV fistula/graft and taped in
place. In some patients, an existing vascular port (e.g., internal jugular vein/subclavian vein) can be used
to connect the dialysis machine instead of an AV fistula/graft. Blood is drawn slowly and sent through
the veins to a filter made up of a series of membranes. Membranes filter waste products from the blood
and transfer them to the dialysate. "Black" dialysate is released from the filter and "clean" blood is
returned to the body through the second needle. At the end of the treatment, the needle is removed. The
dialysis process takes approximately four hours to complete because blood must be slowly collected,
filtered, and replaced.

Peritoneal Dialysis:

Peritoneal dialysis (PD) is a procedure that uses the lining of the stomach (called the peritoneum) and a
detergent called dialysate to cleanse the blood

Desired benefits:

• Provide another way to reduce kidney function.

• It removes waste products from the blood. >• Remove excess fluid from the body.

• Improve quality of life

.
• Dialysis symptoms:

• Drowsiness.

• fatigue.

• shortness of breath.

• swelling (edema) in the body

• Anemia causes weakness

.
• Loss of appetite.

• Drowsiness
.
• fatigue.
DIALYSIS MACHINE

Dialysis mixes and regulates dialysate fluid. Dialysate is a fluid that helps remove unwanted waste from
the blood. It also helps maintain proper levels of electrolytes and nutrients in the body. The device can
also monitor blood flow outside the body. You can hear the alarm sound at the appropriate time. This is
the machine's way of telling us we need to check something.
TIME AND MOTION STUDY
As per NABH guidelines

Time study is defined as a measure of performance used to record hours worked and the cost of work for
specific tasks performed under specific conditions and analyze data to determine the time required to
work at job level.
or it means dividing the work into important elements or simple operation of method work or
process in order to eliminate bad things. After analyzing all the actions in a project, process or job,
it finds the best way to work and complete the work or work.

Purpose
1. They eliminate unnecessary movement, fatigue and try to make people productive.
2. They bring improvements to the business, processes, methods and procedures.
3. They use materials, machinery and human resources well.
4. They also improved processes and created facilities, equipment and facilities.
BENEFITS
1. It can improve the use of materials, construction production, business and money.
2. Ability to define operational requirements.
3. The business can be a model.
4. Developing a work method and comparing many aspects of the time required to
complete a task with the time required to complete the same type of work.
5. Effective cost control and accurate planning can be achieved with the help of time and
research.

HEALTH CARE TIME AND MOTION STUDIES

Measure clinical staff time and movement to learn and monitor efficiency and effectiveness.
For physicians, a number of programs have been initiated to increase the percentage of use by changing
the physician who provides direct care to the patient.
Before the intervention, physicians spent approximately 20% of their time on direct care.
After a major impact, some hospitals doubled this number, some even exceeded 70% of working hours,
resulting in fewer errors, issues and losses.

Determine how long treatment will take through research and action. A specific operation, process, or
mechanical operation. Some of these studies are reported in hospital clinics and OPDs.
The current path to improving the performance of various technologies has led to an interest in
motion and time studies.
The terms "learning time" and "science" have been subject to many interpretations since their
birth. Time studies initiated by Taylor are used primarily for measurement; Movement studies
developed by the Gilbreths are often used to develop methods, but other groups have looked at time
studies as a way to dimension the work of DA.
They use stopwatches as the type of time. Another well-designed zigzag motion study design, it is
easy to determine the right way to think and work.
Today, the debate about comparing the benefits of using one or the other of the two technologies is
over. Industry sees motion science and time science going hand in hand, as evidenced by their
combined use in various fields. Be aware of current trends and be aware of the fact that search
engines are always moving ahead of schedule. Term studies can be used for two purposes:
(1) to help determine the most effective study methods;
(2) to help train people to understand the importance of the time principle and, when training is
sufficiently completed, to enable them to master the use of the time principle code. Today, India's
hospital system is changing;
Hospital outpatient services face major challenges including technological changes and
compensation, public opinion, competing financial needs, and workforce shortages. This is also a
special time as India is in the midst of one of the largest medical and rehabilitation programs in
history. Rethinking doctors' knowledge and skills will influence the future work and performance
of medical services. As part of the Indian healthcare system, significant changes are needed in
outpatient care to ensure that outpatient services are safe and affordable.
The time spent on the auditor's report should be recorded and a description of the report should be
included in the annex of the document. Time and motion studies are studies that record hours spent
on various activities. The applications in the study were carried out in specific narrow areas such as
children's initial registration and food evaluation.
The results must be accurate because the mothers in the study recorded how much time they spent
on certain activities.
Revenue from special cases (e.g. existing/new names) will be estimated until the average duration
is used, but even if there is more duration this value will be used in the calculation.
.Time and motion research in surgery is a better way to identify where hospital capacity and time can be
improved, especially in terms of efficiency and production.
This exercise is the right answer to determine the "time" required to complete a task in a particular
department. Time and research inform policies and help develop effective and robust plans that allow
hospitals to operate more efficiently. Principles on how to understand time and take action include: •
Focus on current tasks/current events
• Identify good action opportunities
• Update the process you are currently doing
• Check if it is producing the desired results
• Rinse and repeat Time And Action, a It is a scanning process used to determine the time and duration
of a task or process.
It is a performance measurement technique used to record and analyze working time and product costs
for tasks determined for the period. Research helps you reduce and control costs, increase productivity
and motivate your employees. Slow machines, personal needs, fatigue and other external factors are also
taken into account when estimating the time required to complete a task. Actually, this work is divided
into several parts. Write down how long it will take you to complete each section. In addition, the site is
classified or rearranged according to the work done. Periodic structures are an important part of the
study of time and space. Time standards are determined by three factors: skilled workers, routine tasks,
and special tasks. The study period includes
: • Learning objectives.
• Test design.
• Regular data collection.
• VIEW information.
The data collection process should be completed in different areas depending on the purpose and nature
of the study. Time and motion data can be obtained using a stopwatch, PDA or video recorder. Many
specialized software packages can instantly turn a handheld or computer into a research tool. As an
option, time and motion data can be collected from the memory of a computer controller (e.g.
computerized time studies).
However, time and motion studies are important tools to increase efficiency and productivity in
healthcare.
