    An incident report details two occurrences where aluminum beads fell from construction on the 8th floor of the Amaia Skies Cubao project. In the first incident, a 557mm bead fell and hit the 6th floor. In the second incident, the bead was caught before it fell. To prevent future incidents, slings will be installed on tools used near the perimeter and netting will cover the south side of the building.     Subject: INCIDENT REPORT- Fall 557 Bead

    Sir:

    Good day.

    An Aluminum Bead measuring 557mm fall from Rm. 834 to 6 th flr. , at around 9:00am. Mr. Edrian Goden was trying
    to fix the bead when it release and fall. This incident happens again at Rm. 838 at around 10:30am, fortunately
    Mr. Goden managed to catch the bead and the rubber mallet.

    To prevent the tools from falling, sling was installed in every tool that will be used on the perimeter. Right now
    MDC is covering the South Side with net and will serve as protection from falling object. In addition to this instead
    of requesting for additional manpower that will serve as a watchman, we are requesting for caution tape so that
    we can isolate the area on 6 th flr where work activity is to be done. As per advice MDC will allow as to use their
    signage.
