ISSUE:

Loss of Common I/O Alarms and Trips.

Some Mark V control panels are experiencing an intermittent, short duration loss
of common IO condition, which is causing nuisance alarms, startup trips, and
other effects due to responses by external systems (DCS) to the reported alarm.
Some of these events are accompanied by a C core diagnostic alarm from the
TCDA board, entitled “IONET failure – relays dropped out”.

RESOLUTION:

While the root cause of this short duration communication issue has not yet been
determined, the following modification to sequencing and firmware is supplied to
reduce the occurrence of unnecessary alarms, trips, and other actions. Normal
response to longer duration or non-transient faults is unchanged.

This modification should only be applied to sites reporting control problems


related to this issue. All sites modified should be properly documented under
PAC.

To implement the prom upgrade option, you will need to order appropriate proms
to upgrade the panel cores, and then install the proms and support files per the
attached installation instructions. Refer to product service bulletin TB01433 for
other details.
