5. In case of Withdrawal of Accumulated Pension Wealth by Claimant/Nominee due to the

death of a subscriber, the Claimant/Nominee needs to submit Death certificate in original
of the deceased subscriber.

6. All enclosures are to be signed by the employee and counter-signed by the DDO of the
office.

7. All documents submitted must be in duplicate.
