Total Added Patients:

In this category we will keep all those patients which are added on 2C platform anyhow.

Total Eligible patients:

In this category we will add all those patients which qualify the CCM program as per CMS guidelines.
Total eligible patient’s numbers/data will fluctuate every month. It will depend on daily
appointments/new patients added/daily CC’s calls. If a CC enrol the new patients in CCM program
the Eligible patient’s numbers will increase and on the other hand if a enrolled/existing patient
declined the service then the number will fluctuate.

Total eligible patients will be count on the behalf of CCM status and CCM status would be Not
Started and Active of particular clinic.

Total Contacted patients:

Total contacted patients would be a number of called patients by CC in current month. Doesn’t
matter what was the CCM status, we just follow the added minutes on 2C of particular clinic.

Total Completed patients:

Total Completed patients would be a number of called patients by CC in current month with at least
20mints documented on 2C. For completed patients, CCM status should be Completed and
confirmed by Care Manager.

Total Call Not Answered patients:

Total Call Not Answered patients would be a number of called patients by CC in current month with
the CCM documented minutes less than 20 of particular Clinic and the CCM status listed as Call Not
Answered by CC.

Total Declined patients

Total Declined patients would be a number of called patients by CC in current month with the CCM
documented minutes less than 20 of particular Clinic and the CCM status listed as Declined by CC

Total HHC/Other patients

Total Declined patients would be a number of called patients by CC in current month with the CCM
documented minutes less than 20 of particular Clinic and the CCM status listed as HHC/Other by CC.
