We are the Senior High School Students from Grade 12 – STEM – Isaiah and at present, we are
conducting the research study titled “The Experiences of STEM Students on Blended Learning at Santa Rita
College of Pampanga.” The objective of this study is to fulfill the final requirement in our subject Practical
Research 2.

We humbly seek your approval if we can administer a close-ended questionnaire, providing


questionnaires/guide questions for the selected Grade 12 students. Participation in this study is completely
voluntary and will only take approximately 10 minutes.

Attached is a copy of the questionnaire/guide questions for your reference and approval. Rest assured

of Grade 12 Students on Blended Learning at Santa Rita College of Pampanga” For the sole purpose of determining
the different experiences of the Grade 12 Students during Blended Learning. In line with this we are inviting you to
be a respondent to this research and request you to spare a few minutes by answering the following
questionnaires/guide questions honestly.
