 Usually is a letter or memorandum or any one of many prescribed or used day-by-
day police operations.

How are Police Reports
categorized?
A. OPERATIONAL REPORTS
 Include those relating to the responding of;

*and a mass of miscellaneous reports necessary to the
conduct of routine police operations.

B. INTERNAL BUSINESSS REPORTS
 relate to the reporting necessary to the management of the agency
and include;
* Financial reports,
* Personnel reports,
* Purchase reports,
* Equipment reports,
* Properly maintenance reports,
* and general correspondence.

C. TECHNICAL REPORTS
 Presents data on any specialized subject, but
usually relate to completed staff work and add to
the specific knowledge, necessary to proper
functioning of police management.

Furnish intelligence information necessary
to the solution of crime accident and police
administrative problems.

Spot report for a violation of law
 is a document that provides immediate and concise information about an observed or reported
violation of law.
A SPOT REPORT FOR A VIOLATION OF LAW USUALLY INCLUDES THE FOLLOWING
INFORMATION:
• INCIDENT DETAILS
- This includes the date, time, and location of the violation.
• Description of the violation
- A brief summary of the violation, including the specific law or regulation that was violated.
• Parties involved
- The names, descriptions, or any other identifying information about the individuals or
entities involved in the violation.

- Any witnesses to the violation and their contact information, if available.


• Evidence

- A description of any evidence or supporting documentation related to


the violation, such as photographs, videos, or documents.
• Actions taken

- Any immediate actions taken by the person reporting the violation, such

1. Reference:

a. Warrant of Arrest and Police Blotter Entry Number 0162 dated April 10, 2023.

2. In connection with the above references, request be informed that on about 2:35 PM of this date at Barangay Bay-Bay, Carrascal Surigao
Del Sur, joint elements of this station and PIB Carrascal PPO led by the undersigned, had arrested one Andrei y Ferrer 50 years old, married, high
school graduate, farmer and a resident of Barangay Gamuton, Carrascal Surigao Del Sur, by virtue of Warrant of Arrest for a crime of Violation
Section 28 of RA 10591 with Criminal Case No. SCC-11041, issued by HON JUDGE LAWRENCE HIDALGO, Acting Presiding Judge of Regional
Trial Court, First Judicial Region Branch 13 Barangay Telaji,Tandag City dated April 5, 2023 with recommended fixed bail bond of One Hundred
Twenty Thousand Pesos (PHP 120,000.00).

3. Arrested suspect was duly appraised of his constitutional rights best understood by him and now under custody of this station for proper
disposition.
