Assessment 1 Summative Assessment

During Mr. Nice’s lesson plan he has given several evaluations and assessments that the supply
teacher will be doing. The supply teacher will be using a work sheet that will be used first as a formative
assessment to check students’ prior knowledge to see where Mr. Nice needs to put more emphasis. At the
end of class, it will then turn into a summative assessment. The supply teacher will give them 10 minutes
to go back over it to fix any mistakes they might have. At the end of class, they will pass it to the supply
teacher which then will be left for Mr. Nice to mark. The work sheet contains 8 different types of Moon
phases. Students will have to identify if it is a waxing or waning phase and then give the measurement.
Mr. Nice has set up the work sheet, so all the students must draw a line from the Moons to the correct
phrase in the word box. It looks like the following.

Rationale 1
The reason why Mr. Nice is giving this handout to the supply teacher is because he wants to see if
the format of the class is beneficial for all learners in his class. He wants the classes to be engaging and to
meet all students’ learning needs. This is extremely important to him since he was one of those students
whose learning needs were not met in the early years of school. This assessment will give students time to
grow throughout the class. They do this by trying it at the beginning of class and then also coming back to
fix or correct any mistakes that were made. The tools that will be used are an answer key. There isn’t any
writing involved so when marking the handout, it will be either a checkmark or a x.
Assessment 2 Formative Assessment
Another assessment/evaluation that the supply teacher will be conducting is a C4U. The C4U
activity that the supply teacher will be doing is called the Oreo moon phases. Tell each student to link up
with their neighbor beside them (Two to a group), hand out a baggy that has been packaged already and
hand out to the pairs. Explain to the groups that they need to decide in the group who is chosen to create
waxing or waning of the moon development. Once they have decided, they can start creating the moon
phases. They will use their fingers to shape the icing from the cookie to create the moons phases.

Rational 2
Walk around and assess each student to see if they remember the material. This activity is for the
students that learn visually and hands on. It is also for the students that need to get and move around to
burn some energy off. This will give the supply teacher time to go around the class to see who needs more
explanation. This will be done as a formative assessment. I have asked the supply teacher in the lesson
plan to write notes on how this was conducted and how it went.

Assessment 3 Formative Assessment


The flashlight investigation will be conducted halfway through PowerPoint. Moon Flashlight
Investigation will be done with a flashlight at the back of the room. Use the cut-out cardboard circle
provided on Mr. Nice’s desk. This will represent the earth. Have a child hold the moon ball in front of the
flashlight. The ball is the moon in this case. Move the circle in front of the flashlight to show the
relationship between the moon and the sun making the phases. Get the students to observe how you’re
moving the circle (moon) to represent it in real life. Let them give it a try for themselves.
Rational 3
The reason why Mr. Nice believes this is important to go over with the students is so they can see
in real time how the movements of the earth and moon affect how the phases come to be. Mr. Nice
believes this is also important for the students who are visual and hands-on learners. This gives visual
learners a chance to learn and understand the material better. Mr. Nice has also planned this activity near
the halfway mark through the class for students to get up, stretch and get excited for the second half of the
class. The only tools that will be used are a rubric with student’s names on it and 3 categories for
evaluation. This is a formative assessment, so this is only feedback for Mr. Nice to see where each student
is at. Below is an example of what will be used and feedback.

Assessment 4 Diagnostic Assessment


This assessment will be done out of class and will be for bonus marks towards other assignments
in class. The assessment is on the 8 phases of the moon. Students will use their science Journals and
conduct their own investigation of the moon phases during the month cycle. They will be instructed to
draw a picture of the shape of the moon and write the date. Mr. Nice also wants the supply teacher to ask
the students to write down any questions or statements they might have. The supply teacher will not need
to pass out anything for the kids to take home. Mr. Nice sends weekly newsletters letting the parents
know what is going on in the class. He has given instructions to the parents already. The Mr. Nice will
continue to remind the students all month long to keep up on their recordings. It is extremely important to
capture the 8 phases.

Rational
The reason Mr. Nice wants students to take this home is that he wants students to connect what
they are learning in the classroom to real life events/situations. This is a good way for the students to
explore and ask additional questions and bring them to class. Parents will be able to see that their children
are learning and engaged in and out of the classroom as well. Mr. Nice also wants the students to
investigate and use critical thinking not just in the classroom but outside of it as well. There won’t be any
tools used to mark the journals. The goal here is to have the kids explore on their own and it will be
simple if they put effort into it then they will receive a check for completion.
