AFFIDAVIT

We Nikki Ann Reyes, a cashier and a resident of Lawaan 1,

Talisay City, Cebu, Octavio Oropesa,an assistant cook, and a resident
of Lawaan 1 Talisay City, Cebu, Orlando Pata, a Head Chef, and a
resident of Pajo Lapu Lapu City, Cebu, and Edeliza Cortez, a cashier,
and a resident of Biasong talisay City, Cebu, all of legal age and
employees of Adelante Pizza Place, Under oath, state:

1. THAT We are all employees of Adelante Pizza Place;

2. THAT Adelante Pizza Place started operating in the year

2021.
3. THAT on that year none of us have been employed yet

4. THAT We, Nikki Ann Reyes, Octavio Oropesa, Orlando Pata,

and Edeliza Cortez, started our employment only on May 20, 2023,
September 08, 2022, June 10, 2023, and September 17, 2022,
respectively;

5. THAT the information stating that our SSS contribution

commenced on the year 2021 is erroneous;

6. THAT the error can be attributed to our supervisor whom

our employer entrusted the task of processing our SSS application.

7. THAT such error was purposefully done in order for the

employees to be able to apply for a loan in the SSS.

8. THAT We are executing this affidavit to confirm and affirm

the foregoing facts and for whatever legal purpose it may serve
