    AFFIDAVIT OF KINSHIP

    I, ARLENE PREMACIO SATO, of legal age, married, and presently residing at San
    Fernando, Cebu, after being duly sworn to in accordance with law, hereby depose and say
    THAT:

    1. I am one of the surviving nearest kin, being the sister of CESAR PREMACIO JR. who
    was born on October 20, 1960 and died on April 6, 2022 at San Fernando, Cebu;

    2. That both our parents are already deceased;

    3. the decedent has no surviving spouse and children;

    4. That I am executing this affidavit to attest to the truth of the foregoing facts for the
    purpose of claiming my brother’s death certificate from the PSA.

    IN WITNESS WHEREOF, I have hereunto set my hand this 28th day of July 2022, at
    Cebu City, Philippines.
