I, SITTI RUBIN ADHELFA DE ARCE, minor, Filipino, and presently residing at

Block 20, Lot 12, Malesido Homes Phase 1 & 2, Barangay San Isidro, General Santos
City, after having been duly sworn to in accordance with law, do hereby depose and
say:

1. That my biological father is DANILO MAGBANUA PAJEL JR., a person of

legal age, Filipino, and presently residing at Block 20, Lot 12, Malesido Homes
Phase 1 & 2, Barangay San Isidro, General Santos City;

2. That I was born on January 21, 2007 at General Santos City, Philippines;

3. That the facts of my birth were duly registered at the Office of the Local
Civil Registrar of General Santos City;

4. That I desire to be the legal and adoptive daughter of DANILO MAGBANUA

PAJEL JR., thus, I wish to be his child and legal heir;

5. That I wish to use the surname “PAJEL” to be my legal and registered

surname in my Certificate of Live Birth; and

6. That I execute this affidavit to attest to the truth of the above facts in
compliance with the provisions of Republic Act No. 9255, as amended.

USE OF FATHER’S SURNAME

I, ARLENE BARLIZO DE ARCE, of legal age, Filipino, and presently residing at

Block 20, Lot 12, Malesido Homes Phase 1 & 2, Barangay San Isidro, General Santos
City, after having been duly sworn to in accordance with law do hereby affirm, under
oath:

1. That I am the biological mother of SITTI RUBIN ADHELFA DE ARCE;

2. That I personally assisted my daughter in the execution of her “ AFFIDAVIT OF

PATERNITY AND USE OF FATHER’S SURNAME ” which was executed on May 19,
2022 docketed as in the notarial register of ATTY. LEO ANTONI C. ESTANIEL;

3. That I further affirm that my child SITTI RUBIN ADHELFA DE ARCE signed the
said affidavit in my presence; and

4. I affirm all such in order to inform the authorities concerned on the truth of the
foregoing facts and for whatever legal purpose it may serve.
