AFFIDAVIT OF RENUNCIATION OF FOREIGN CITIZENSHIP

I, GUNASEELAN SANCHEZ, of legal age, Filipino, single, and a resident of

Mother Ignacia Village (MIV), Pulpogan, Consolacion, Cebu, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state that:

1. I was born on October 6, 1996 at Sabah, Malaysia;

2. My parents are ______________, a Malaysian, and ________________, a Filipino citizen

at the time of my birth;

3. I am seeking public office in the Philippines as a member of the Sangguniang

Kabataan (SK) in Barangay Pulpogan, Consolacion, Cebu this coming May 14,
2018 barangay and SK elections;

4. I hereby voluntarily and expressly renounce my Malaysian citizenship, as well

as any and all foreign citizenship or allegiance;

5. I am executing this affidavit of my own free will for whatever legal purpose
this may serve.
