Fidelity Bond ApplicationForm (Generated through OFBS), submit with attached 2x2 photo and signatures
List of Bonded Accountable Officers
Oath of Office by Barangay/SK Chairperson
Duly Notarized Resolution for Appointment of Barangay/SK Treasurer
Approved Annual Budget and Barangay Appropriation Ordinance

Additional requirements for KALAHI Funds


PPMP and Program of Work
Barangay Resolution Indicating the signatories of the account opened for KALAHI
Certification/MOA from DSWD identifying the Barangay as Recipient for KALAHI
For Area Coordinators and/or BDC-TWG - certification of duty/position

Applications must be scanned and submitted via email (surigaodelsur@treasury.gov.ph), with email
subject - Fidelity Bond Application (Name of Agency)
