ENGINEERING CONSTRUCTION REFERENCE SHEET
INSTRUCTIONS:
1 The Construction Reference Sheet must be completed in its entirety.
A Construction Reference Sheet must be submitted for each category/specialty code being requested. Feel free to make copies of this

form as needed.
A minimum of four references must be supplied for evaluation for each category/specialty code. The maximum number of categories

that can be approved is three. Dates of projects must be in the following format (mm/dd/yyyy).
The same references are permitted for different specialties so long as the agency/company supplying the reference can attest to that

specific category of work.
References are required upon recertification. For existing categories, references can be for work completed within the past 5 years. If

pursuing a new category, references should be for work completed within the past 3 years.
NOTE: The contract for each reference must be $100,000 or higher.
