Clinical summary

Many thanks for your referral. This patient was reviewed in the telephone clinic in view of the COVID pandemic.

She has symptoms for last 8 months she has change in bowel habit. She has developed nausea and vomiting associated with
shivering over the last few days and has also developed a painful lump in her back passage which is impeding her mobility. Her
bowels open 2-3 times per day with formed motions but not blood PR.

PMH - PID

She has had 4 courses of antibiotics over the last 4 months. A colonoscopy earlier this year was normal. She tells me she was
also admitted in March this year to hospital with pancreatitis, gallstones were excluded.

I have arranged an MRI Small Bowel to exclude small bowel pathology. However, I have advised her to attend A&E today
regarding her rectal pain, vomiting and shivering as she may have an abcess.
