A 70-year old man presents at the Accident & Emergency Department with a history of increasing
breathlessness over the past few days. He is a lifelong smoker and five years previously suffered an
uncomplicated inferior myocardia1 infarction. He is a retired electrician who had received extensive
asbestos exposure during the construction of Didcot Power Station. He produces a small amount of
sputum each morning, and over the past few days this has become discoloured. When you examine
him you observe him to be extremely unwell.

(A) What three physical signs would you elicit to determine whether he is in established

After appropriate treatment he recovered from his acute respiratory illness over the next ten days in
hospital. Before discharge you perform lung function which reveal an obstructive defect of
ventilation that is partially reversible with a bronchodilator. In addition, there is increased total lung
capacity and residual volume, with a reduction in gas transfer.

cardiac failure. Her full blood count shows haemoglobin = 8.lg/dlYMCV = 106 fly platelets = 90 x
109/L, white blood count = 4.0 x 109/L (normal differential). Other investigations show Na =
136 mmol/l, K = 3.8 mmol/l, creatinine = 80 pmol/l, AST = 45 IUA, bilirubin = 32pmol/l , alkaline
