Undersigned is respectfully requesting for the Untagging of Abandonment in the E2M system of the
shipment consigned to NAME OF COMPANY. covered by Bill of Lading No. _____________ which arrived at
the Port of MHCPSI on November 23, 2023 on board via vessel MV _____________ with Registry No.
________ and in support under oath thereof, hereby deposed and state that;

1. The shipment was tagged as “abandoned “ in the E2M system due to the processing of
Cancellation of SAD was only finished on _____________________.

2. Thus, the request for Untagging of Abandonment files under Section 3 of CMO 24-2015;

3. The request for Untagging of Abandonment is not intended to violate or circumvent any law,
rules and regulation.

4. The consignee under rules to take full responsibility for whatever consequence or liability that
may result from the approval of the request and the release of the shipment.
