State Govt. nor is being funded by any other organization to ensure that there no duplicity of
the CSR activity under reference.

Career Plus Educational Society or the organization itself does not have any material or
pecuniary relationship of transactions with Delhi govt health department, its Promoter, its
Directors, its Senior Management or its Holding Company, its Subsidiary and Associates which
may influence the process of administrative approval or financial concurrence or fund
disbursement for the proposed project.

5. We, Career Plus Educational Society assure that if Health department Delhi state
approves any financial support towards proposed project;

a. will submit a Fund Utilization Certificate to health department, issued by a Chartered


Accountant as per the given format at Annexure-VI of project Compliance Checklist.

b. will retain all relevant documents like Cash Book, Bank Book, Ledger, Journal, Relevant
Bills, Vouchers and Receipts etc. for a period of at least 03 years from the Project/Event.

c. That during this period of 3 years agency will allow authorized representative(s) of
health department for audit (s) / inspection(s) of above documents as and when asked by
Delhi govt.

d. That during the period of 3 years as mentioned above, on health department demand
agency will provide the desired documents/ information pertaining to income & expenditure
detail of the project/ event including copies of relevant Bills, Vouchers and Receipts etc. for
internal and/ or external purposes (for supplying to 3rd party) as per law.

1. Executive Summary

This proposal is drive with a vision to alleviate diseases like malaria , typhoid ,dengue and
other water borne diseases during monsoon season through developing platform for Heath
checkup camps and diagnostic service along with doctors consultation, providing medicine
&reffering serious patients to govt hospitals development for the community living in slums of
East & north east delhi ( near to yammuna banks) .
Health plays a very important role in development of any country, but health alone does not
promise the overall socio-economic development of any individual. Today the near by low lying
areas of yammuna got effect by the flood crisis due to which lot of water borne diseases are
likely to occur . To prevent those disease to affect the local community living near by the flood
effected regions.

Need Assessment

Wednesday, July 12: Relentless rains first began to pound the national capital over the
weekend, when Delhi ended up recording more than 150 mm of rainfall in 24 hours, the
highest in 40 years. But even now, days after the downpours ceased their violence,
Delhiites continue to helplessly watch as flood waters slowly inundate the city’s streets.

Despite its familiarity with waterlogging during the rainy season, the national capital rarely
faces full blown flooding. However, this time, the threat of a major flood looms over the
city, and we have the swelling Yamuna to blame.

Water-levels in the Yamuna first crossed the danger mark of 205.33 metres on Monday
evening after the Haryana government released 3 lakh cusecs of water from the
Hathnikund barrage. And things have only gone further downhill since.

As per the Central Water Commission's flood-monitoring portal, the water level at Delhi’s
Old Railway Bridge crossed the 207-metre mark at 4 am on Wednesday (the first time since
2013), and quickly rose to 207.55 metres by 1 pm. This means that the Yamuna water levels
in Delhi have breached the all-time record of 207.49 metres set in 1978!

Further, thousands of people residing in the floodplains have been evacuated and shifted
to safer places due to rising levels of the Yamuna river. These low-lying areas along the
river, considered vulnerable to flooding, are home to roughly 41,000 people.

Officials said the teams in six districts — North, North East, Shahdara, East, Central and
South East — were rescuing people and rehabilitating them in schools, community centres,
night shelters and vacant plots and buildings. As of Tuesday, 2,700 relief camps housing
approximately 27,000 individuals had already been established.
Apart from the standard disaster management preparedness such as constructing artificial
barriers to prevent the breach of water from the Yamuna and installation of pumps to
manage water levels, officials have also issued alerts for public safety.

This figure shows that number of laboratory technicians are not enough for diagnosis of
diseases, hence people have to go to cities to access diagnostic services which increases the
cost and time. Thus, intervention is required in improvement of Health Care Services in rural
areas of India

At the same time, corporate under Corporate Social Responsibility can play a very important
role in diseases alleviation. Corporate Social Responsibility (CSR) is the responsibility of the
corporate entity towards the society in consideration of the support given and sacrifices made
by the society.

the community to ensure healthy and better quality of life.

3.2 Health Camps process at Career Plus Educational Society :

The training activities would be broadly divided into three major phases, which
are following:

a) Preparatory phase - Preliminary Market Survey, selection of Slums, procurement of


Medical camp infrastructure, networking with from potential Laboratories, selection and
train the Doctors & medical staff (T3) etc.
Mapping the local areaswhere probity of communicable disease are high after the flood
( Yamuna banks)under nourish women, elderly people & infants.
b) Mobilization and Outreach - The BPL people will be selected from the effected areas with
the assistance of the published data of the BPL persons and in coordination with DRDAs
and local bodies. Local advertisements, nukkadnataks, road shows, tie up with local
NGOs will help us ensure the same. A well equipped local team will be responsible for
identifying the areas and providing them the medical assistance, counseling them and
disciplined living with the help of local NGOs and relevant stake holders.
c) Delivery Phase: Induction of Medical staff, sample testing , doctors consultation&
providing medicine, health tracking, liaison with potential Hospitals govt for serious
patients admission .
Camps and mobile nutrition vans along with nutritional experts , will run awareness
programme in association with asha workers & provide help to the above mentioned
target audience.

I. Preliminary Market Survey
A Preliminary Market Survey is always done to assess the potential Medical support
requirement in the local area, post flood health situation monitoring, etc. This
assessment forms the basis of finalizing the Camp location and methodolgy according to
the market demand. The assessment would be repeated after 2 months to validate on
the progress, health situation if needed. Chosen camps would be continued in subsequent
batches till reasonable numbers of people as per potential assessment are medically fit.
Mobilization of Doctors & health workers: The Doctors, health workers & Medical
supplies are selected from the targeted project area (rural/ urban) with the assistance of
the published data of the BPL persons and in coordination with DRDAs, asha workers and
local bodies. Local advertisements, nukkadnataks, road shows, tie up with local NGOs will
help us ensure the same. A well-equipped local team is set up and made responsible for
identifying the community and counseling them to orient themselves towards changes in
the environment and disciplined living with the help of local NGOs and relevant stake
holders. Team also undergoes a detailed training program which familiarizes them about
the goals of the program .

II. Identification and Selection of Flood affected public through Candidate
Profilers: Mapping medical Needs and Suitability to the Local Population is a crucial step
in project execution. The identification of Medical beneficiaries would be based on the
following criteria: Beneficaries in the allage groups standard as required for the Medical
benefits are selected from rural BPL families, as per the list maintained by the District
Rural Development Agencies (DRDAs)..
Local Asha workers , DRDA, panchayats& Health bodies will provide data of malnourished
females , elderly people & infants.

Public walk-in through the camps once the check up is announced after the CMD
exercise, the selection of Health workers is done through an Entry gating test known as
PROFILER.
The Profiler facilitate in identifying the hidden or key skill areas of the candidate through
the by testing and segregating under 4 categories viz.

 Qualification Mapping: The community health are mapped with the mapped samarth
profiler and with the counseling results.

 Camp Allocation: Final allotment of camps is done after the qualifications and
aspirations are mapped as per the “Samarth Health camps”unique profiler.

III. Health Camps:


Dedicated Centers are established as required and a network of BPs are created to
strengthen the same. We may seek the help of service providers to strengthen the
outreach in terms of infra. Samarth Health camp centers will be equipped with
appropriate medical aids and tools so as to enhance the efficacy of check up& testing.
Camps Partners: Partnership will be established with healthcare industry
representatives / business partners that have the aptitude and capability to conduct
testing, so that the comparative advantages of the partner can be leveraged to improve
the implementation of the programme.
IV. Content:
camps modules are selected to suit the needs and demands as per the opportunities
available in the local market and context. A survey has been done for a number of
districts of the states. The PMS includes collection of information from all stakeholders
and sources like industries, business houses, trade unions, government agencies,
technical institutes etc. The PMS will be a continuous process to ensure dynamism in the
project.,
Health workers selection
Career Plus Educational Society follows a diverse approach for health workers selection,
which is based on combination of required skill set and relevant industry experience for
conducting samarth health camps.

Each trainer position has a specific job description document which is used to baseline
the skills set and initiate hiring process. Competency grid forms a critical part of this
document which entails the required functional and behavioral expertise.

 Candidates with matching profiles having relevant experience and/or inclination


towards training delivery are sourced using various channels and are enrolled for
online examination which acts as first elimination round.
 These examinations are specific to industry and are designed by subject matter
experts.
 Candidates who clear the online test are personally interviewed by experts followed
by standup delivery session, either face to face or over SKYPE for remote locations
on relevant topic chosen by the candidate.
 Ratings from stand up sessions are then mapped using the IEF (Interview
Evaluation Form) by more than one SME to ensure selection of the right profile.
 Selected candidates go through a final round of interview with Capability Building
Leader before hiring is closed by HR.
V. Training the health workers (T3) process at Career Plus Educational Society

At Career Plus Educational Society T3- delivery of training program is headed by Master
Trainer for that subject, who is a Subject Mater Expert and is closely involved in selection
of each candidate. Master trainer formulizes the course outline and content in
collaboration and direct support of content development team. The course duration is
decided as per the training objectives and course content which involves identifying the
right mix of training methodology for effective delivery.

Train the health workers modules (T3) are prepared for all course conducted and
facilitated at Career Plus Educational Society to ensure standard delivery framework at all
touch points. These modules act as enablers to the delivering faculty and maximize the
potential by providing common platform for certification. The measures used for deriving
certifications are based and aligned using industry benchmarks.

These modules are designed with a clear objective of facilitating adult learning by
collaborative approach of transferring skills and knowledge. Career Plus Educational
Society follows an indigenous approach in designing T3 modules using structured
market/industry research articulated by industry experts. Industry expertise is combined
with platform skills to create a learning experience for participating adults, resulting in
strengthening participant’s domain skills. Modules are designed with clear focus on the
nature of the project and/or prevailing socio economic environment which makes it
assuring for trainers to connect the participants with clear objectives.

and capability. To ensure quality and standardized delivery of the program across the
country, each of these selected trainers will undergo a specially designed “train the
trainers” (TTT) program. All the trainers undergo refresher courses periodically.

T3 programs are delivered using the best suited mix and methodologies. Most of the
courses follow a hybrid and a need based approach, which includes:

 Pre Read/Self Study
Best combination from the modes above is used to ensure effective learning resulting in
transferring of skills over to faculty and thus enabling them for quality delivery. The
training of health workers will be completed within the 10 days.

VI. Testing & Medical supplies:


The Testing at entry is based on the certain checklist comprising of basic minimum
standard. And then there are other parameters like water borne diseases, Malaria, and
Typhoid. As shown in the following diagram, major stages will include:

Pre-Health Assessment :
After the final allotment of the camps basis counseling and “Samarth health camp”
profiler, we do a pre health assessment to gauge the level of health situation. This help in
bringing everybody at one platform.

Mid- blood testing :


As all do not have the same levels of concentrations, it is natural that all individuals
undergoing check up will be at different levels of grasping at any given time during the
camp. Our blood testing is designed especially for the comparatively weaker public who
require extra medical attention and thus do not feel left out. It will ensure the following:
 Identifythe health problem areas of public.
 Help engage better with weaker public to bring them up to expected levels.
 Individual blood testing on basic parameters of the public.
 Restrict time loss.
 None of the indiviual feel abandoned and gain health confidence.

Post-Testing results & Medicine supplies:


To assess if the person is not medically fit as per the testing report:
 Exhaustive formative tests & medicine will be given
 These will include health components and basic water borne disease

Referring to hospitals by doctors:


A robust process at the end comprising all key health areas:
 The testing results will aim to check the health level& serious patients will be refer to
govt hospital for admissions .

Sustainability of the project

The sustainability of the project generally depends on the health demands and necessities of the
comunity. In the proposed sector there is huge demand of health care testing and disease
prevention post flood in the selected areas, where the comunity would be offered medical
assistancewith the Diagnostic Centers & Labs available in the proposed locations. The
sustainability of the program will continue post the project completion, as the probability of the
healthy comunity continuing and growing in their good health will be high.
The program has also inbuilt component which includes 100% health check up with doctors,
pharmacy, health worker and other health care units and by spreading awareness on
benefits of early diagnostic tests
