Dear Dr. Martins

Re: Robert Smithson, aged 22

Mr. Smithson is being discharged from Berkeley Hospital, back into your care today. He was admitted in
the burns unit after being in a house fire and he had suffered Partial-thickness burns and he needs your
further care.

Mr. Smithson presented in Emergency Department with Partial-thickness burns (18 percent of his body)
on his chest and neck, respiratory distress and in his oral cavity was found soot. Upon his arrival in our
ward, he was assessed with chest X-ray, blood tests, cardiac monitoring, all in normal range. An
indwelling urinary catheter was inserted; IV fluids and high-flow oxygen were administrated.

During hospitalization, Mr. Smithson’s was assisted with his burns by running cold water, elevating head
and torso, auscultation to prevent stridor, hoarseness, coughing and wheezing . He also was prescribed
Naproxen 500mg, Tramadol HCL 50mg, Gabepentin 300 mg and tetanus vaccine.

Mr. Smithson requires assistance with wound management and auscultation. He was advised how to
manage his skin by moisturizing, massaging and applying UV protection, performing deep breath
exercise to increase lung capacity, avoiding swimming and contact sports, adopting a healthy diet.

A follow-up appointment was set-up on 17th April 2018 in the Burns Unit Outpatients.
