Re: Mr. Robert Smithson, 22 years old

Dear Doctor,

I am writing to update you about Mr. Robert Smithson, who was diagnosed with Partial-thickness burn
and Respiratory distress. He is set to be discharged today and will be needing continuous monitoring
and care.

On 6 April, Mr. Smithson was brought to Berkeley General Hospital due to burns 18% TBSA on neck and
chest following a house fire. He has been commenced with IV fluids, and irrigation with running cool tap
water on the affected areas. He also suffered from respiratory distress and has been managed with high
flow oxygen. Consequently, he has been stable as evidence by no infection, no fever, and all the
laboratory work ups are within normal limit. He has been commenced with Naproxen, 500mg/tab twice
a day, Tramadol HCL, 50mg/tab twice a day as needed, Gabapentin, 300mg/tab daily before bed time,
and Tetanus prophylaxis.

It would be greatly appreciated if you could continue to clean his wound and apply topical dressing.
Daily auscultation of respiratory status will benefit him. Please note that skin care, breathing exercises,
and recommended diet has been instructed. He has been advised to do light exercise and avoid contact
sports. Lastly, he has an appointment on on 17 April at Burns Unit Outpatients.
