Re: Mrs. Beryl Casey, 72 years old

Dear Nurse Manager,

I am writing to refer Mrs. Beryl Casey, who was diagnosed with Fractured left neck of femur. She is set to
be discharged today and will be transfer to you for continuous care and rehabilitation therapy.

On 4 February, Mrs. Casey was brought to our institution due to fainting after getting out of bed
following fall incident, which prompted her admission and for hemiarthroplasty. During confinement,
she has been transfused with 3 units packed cells, and IV antibiotics has been completed. Also, she was
known to have hypertension and was prescribed with Ramipril, 10mg daily. However, she has episode of
hypotension and her medication was decreased to 5mg daily. Post-operatively, she was able to walk
with pick-up frame in a short distance with 1 assistance, and an abduction pillow was applied when at
rest in bed. In addition, an anti-embolic stockings was recommended to wear for 14 days.

It would be greatly appreciated if you continue to monitor and assess her needs for rehabilitation
therapy on in-patient basis and upon returning home. Kindly monitor her blood pressure for 3 days as
her Ramipril has been adjusted. In case she complained of pain, Paracetamol, 1g four time a day has
been instructed. Please note that her hemoglobin level must be rechecked tomorrow, and staples on
her wound will be removed on 10 February. It would be grateful if you could inform her family of the
transfer today at 11:00am.
