Re: Mr. James Andresen, DOB: 12 September 1944

Dear Doctor,

I am writing to update you regarding Mr. James Andresen, who was diagnosed with Aspiration
Pneumonia. He is set to be discharged today and will be needing continuous monitoring and care.

On 12 January, Mr. Andresen was brought to Prince Wales Hospital due to three days shortness of
breath, productive cough, body weakness and fever which prompted his admission. During confinement,
a sputum M/C/S has been done which revealed Klebsiella pneumoniae infection. Therefore, he has been
commenced with intravenous fluids and antibiotics. In addition, he has been engaged to exercise
programme due to weakness of extremities upon assessment of physiotherapist. Consequently, he has
been able to move using a 4-wheel-walker and 2 person assist with mobility belt during transfer. His
wife also reported of choking during feeding. He has been referred to a dietician for nutritional
management and feeding schedule.

It would be greatly appreciated if you could continue to monitor him upon discharged today. It would be
beneficial if you could encourage him to continue his recommended exercises and dietary regimen. As
he is experiencing constipation for which Sienna is instructed. Please note that feeding techniques and
monitoring of temperature has been instructed to his wife. Lastly, he has follow-up visit from
physiotherapist for 2 weeks and community nurse to visit after 1 week.
