Re: Benjamin, 69 years old

Dear Ms. Bates,

I am writing to update you (known patient) regarding the care of Mr. Benjamin, who undergone a total
hip replacement. He is to be discharge today and will be needing continuity of care.

Mr. Benjamin was diagnosed with osteoarthritis in 2003, which prompted his admission on 5
September. Post operatively, he is apparently well and he was able to walk independently with the aid
of wheelie-walker. During the course of treatment, daily dressing change of his wound was done, and he
was also commenced stretching exercises. Also, his hemoglobin level is 71 and 3 units of packed RBC
were transfused. Currently, his hemoglobin level is now at 112. Additionally, he also has a high blood
pressure and was prescribed Aspirin, 100mg and Ramipril, 50mg.

It would be greatly appreciated If you could continue to assist him on his daily activities. The hospital will
provide the wheelie-walker and a wedge pillow. Also, a raiser was hired for 2 weeks with the help of a
local medical supplier. In addition, he has recommended Panadeine Forte, 6 tablets/day for pain. It
would be beneficial if you could follow up his FBE and UEC test result.
