Dear Ms McKie,

Re: Rachel Brown,

DOB: 12 December 1943

I am writing to refer Mrs Brown, a 76-year-old widower, who was admitted to our ward
following an infected ulcer on her left leg, requiring your help with wound management.

Mrs Brown lives independently in a retirement village. Active person, she is not helped by any
care worker but her daughter lives nearby and is very supportive.

Reluctant patient, Mrs Brown noticed the ulcer two weeks previous to her admission. Her
nursing management consisted of washing her leg, applying four-layer compression bandaging
and monitoring cadomexer iodine dressings. She must complete her course of antibiotic,
oxacillin intramuscular 300mg, every six hours and paracetamol if required, no more than 8 per
day. Her hypertension and hypercholesterolemia medication must also continue.

Mrs Brown has made good progress and she is ready for discharged. She was informed regarding
the importance of wearing compression stockings, ample bed resting and leg elevation. A
dietician to review her diet is advised as she needs to lose weight, to reduce ulcer recurrence.
Please ensure daily dressings with cadomexer iodine and triamcinolone ointment, monitoring
wound progress, daily activities management and an occupational therapist if needed.

An appointment was provided on 25.08.2019 at Community Clinic to check her wound progress.

Do not hesitate to contact me if you have any queries.

Yours sincerely,

Nurse
