Re: Mrs Margaret Maggie Goldstein, aged 61

Dear Sir/Madam,

I am writing to discharge Mrs Margaret Goldstein back into your care. She was admitted in our
hospital on November 8, 2017 due to a dislocated right elbow and your continuity of care is
requested. 31

Mrs Goldstein presented to hospital with the abovementioned injury following a fall in your
nursing home. Subsequently, it was noted that the affected elbow was healing slowly;
therefore, a cuff with sling was applied. In addition, she developed urinary incontinence on
November 10, 2017, for which adult diapers were used.47

During hospitalization, Mrs Goldstein was commenced on Oxycodone 5mg thrice a day,
Atacand Plus 32/12.5mg a day, and Remeron 15mg/day. Regarding her mobilization, she is able
to mobilize with the aid of a wheelchair, while assistance during transfers is required. Please
take note that skin integrity was maintained although she showed reluctancy in using adult
diapers.55

As Mrs Goldstein is hemodynamically stable, she will be sent home for discharge. Kindly
monitor the healing progress of her elbow and provide assistance in showering and
incontinence care. Equally important is to facilitate physiotherapy follow-up which is scheduled
on November 28, 2017, 2pm at Eventide. Lastly, she will be escorted by her husband, Mr
Goldstein to your facility.55
