Re: Mr. George Gale, DOB: 24 April 1936

I am writing regarding Mr. Gale, an 85-year-old widower who was admitted on 10


May 2010 due to a fall associated with disorientation and fever. He is being
discharged today and needs short-term care in your service.

Mr. Gale was diagnosed with UTI recently, which may be related to the cause of
the fall. Two weeks before, he had an episode of vomiting, palpitations, and
dysuria. During hospitalization, he has made good progress, and he has
commenced on IV amoxicillin 750mg every 8 hours for UTI. Additionally, he has
been on his regular medications, which are paracetamol for osteoarthritis since
2003 and felodipine for hypertension since 2009.

Mr. Gale lives alone in his apartment. However, he socializes with his neighbors.
Additionally, he is known to be independent with his ADLs. He drinks occasionally
and smokes 10 cigarettes per day.

It would be appreciated if you encouraged him to do daily physical activity for 30


minutes, as his mobility has decreased recently. Additionally, he still shows
episodes of confusion, for which assessment for independent living is
recommended. Please note that he is keen to return to his own home.
