Dear Sir/Madam,

I am writing to refer Ms. Riya Ray who has recently delivered her first child at Women’s College
Hospital last 22nd of February. She requires short-term emergency accommodation and support
services from you following her discharge today.

Ms. Ray is living with his current boyfriend, Anton Campbell, since August 2017, but he is not
the father of the child. Ms. Ray does not have any relatives near her place, as her parents died
18 months ago, her sister lives overseas and her aunt lives four hours away, but stays in touch
with her through call.

Mr. Campbell is verbally abusive, and therefore, she does not want to live with him. Additionally,
her parents died in an accident and had depression since then, but haven’t seen any treatment
for it. So she was referred to a clinical psychologist for emotional support and counseling
regarding her parents’ death and current relationship problems. Also, she is unemployed and
has no contact with the father of the child, so she has been advised to apply for single mother
benefits.

During hospitalization, Ms. Ray and her baby have been progressing well. However, she is
breastfeeding with hesitation and prefers to change into bottle feeding. It would be most
appreciated if you could provide the necessary accommodations and support to Ms. Ray and
her baby. Liaising with the local Maternal Health Service for ongoing post-natal monitoring is
important.
