Thank you for seeing Ms Hoffmann, a 34-year-old patient, who has signs and symptoms suggestive of
reactive depression and anxiety. Your further investigations and treatment would be highly appreciated.

On 22/11/2014, Ms Hoffmann presented complaining of severe job stress, poor memory and
concentration, loss of pleasure, and loss of confidence. Besides that, she reported that the job stress
caused her depression, nightmares, insomnia, difficulty in getting up, loss of appetite, and loss of libido.
This paragraph has a lot od data and very big one , try not to collect all data in one
paragraph esp. if you have data regarding the patient personal or social life.

By that time, she was living alone, as her boyfriend had has broken up with her recently. Additionally,
she considered quitting her job, for because she wanted a break from working. Thus, temazepam 20 mg-
30 mins before bed was prescribed and a referral to a psychiatrist was advised to which and the patient
refused.

On today’s visit, Ms Hoffmann showed stated low compliance (to what?) as she did not fill her
temazepam script. Despite refusing a referral to a psychiatrist a week ago, she finally agreed .to be
referred after talking her into it again

In view of the above, I would be grateful if you can take over Ms Hoffmann’s care. Should there be any
queries, kindly do not hesitate to contact me.
