I am writing to refer this patient, a 22 year-old single builder who has worsening asthma.

Mr. Foster has been treated from asthma since he was 3 years old with Pulmicort 200 mcg inhaler twice Commented [jc1]: for
daily and Ventolin inhaler on need. Furthermore, he has previous history of hay fever, eczema and is Commented [jc2]: as needed
allergic to cats. He has a positive family history of asthma in his sister. Please note, he is a smoker. Commented [jc3]: a

Mr. Foster presented with exercise induced shortness of breath, wheezes and cough especially at night

over the past 3 weeks with more frequent use of Ventolin inhaler. Moreover, he had a burning sensation

over the lower part of his chest especially after meals. Clinical examination was unremarkable. Therefore,

pantoprazole was prescribed for possible GORD. Chest x-ray and CBC were ordered.

On Today's review, Mr. Foster was non-compliant with Pulmicort. In addition, chest x-ray and CBC were
normal. Therefore, advice regarding smoking cessation options such as nicotine patches ans support Commented [jc8]: and
groups was given. He was also advised to avoid missing or doubling Pulmicort doses. Lastly, pantoprazole
was prescribed for another 7 weeks.

Mr. Foster is referred for lung function assessment and asthma management advice. For any queries,
