Re: Ms Susan Forrest, DOB 19/05/1957

I am referring Ms Susan Forrest into your care for further assessment and evaluation regarding her asthma.

Ms Forrest presented on 25/02/19 consultation with a 2-day history of shortness of breath associated with persistent cough
for 7 days, which demanded a more frequent use of Ventolin. Her PEF was 330 in the ocasion and a CXR and FBC were
ordered.

On review two days after, Ms Forrest reported worsening of the shortness of breath and fatigue. Her temperature was
37,5°C, PEF was 200 and the CXR revealed consolidation in the right lung. The FBC is in processing and a CT scan was
ordered for definitive diagnosis.

Please note that Ms Forrest is an active woman who has asthma since 4 years old. She is a smoker for 7 years, has
allergic rhinitis and eczema, for which she takes Ventolin, Symbicort twice a day, Zyritec and topical hydrocortisone as
required.

It would be appreciated if you could assess Ms Forrest and evaluate for a probable community-acquired pneumonia. There
is also need to review her medications and dosages in addiction to reiforce smoking cessation options.
