Re: Mr George Gale DOB: 24.04.1936

I am writing to refer Mr George Gale, an 85-year-old widower, who was admitted to


Oldtown Hospital on 10.05.2021 with a diagnosis of possible urinary tract infection following
a fall. He is being discharged to your nursing home and require ongoing temporary
management and support.

During hospitalization, Mr Gale has made steady progress, and his vital signs are within
normal limits. His infection has been controlled. However, his mobility has been sharply
delined. It would be appreciated if you could encourage him to undergo some physical
activities for 30 minutes per day.

Mr Gale lives alone in his two level flat without an elevator, and he has no home assistance.
Despite this, he is eager to return his home. I have some concerns about his ongoing
confusion, and I recommended a detailed assessment to determine his suitability for
independent living.

Mr Gale has a history of osteoarthritis and hypertension, for which he takes paracetamol
(500mg 2 tables ,4 times per day) and Felodipine (5mg, once a day). In addition, he
complains of Non-specific colitis that needs ongoing monitoring.
