Re: Michael Stamp, DOB: 28.08.1986

I am writing to refer Michael Stamp, a 35-year-old self-employed mechanic, who I have been
visiting for his regular INR test. He requires an assessment regarding his weight and possible
depression.

Mr Stamp has gained 20kg of weight over the last two years since his wife died, and he has
an unhealthy diet, and is overweight ( BMI 31). He does no exercise as he injured his knee
while playing football one year ago.

He has no relatives or friends in the area, and he has possible underlying depression. He still
misses his wife, but he has never sought professional help or grief related counselling.

Regarding his medical history, Mr Stamp has AF (atrial fibrillation) and diabetes type 1, for
which he takes Warfarin 5mg mane and Metoprolol 25mg BD, and uses an insulin pump. He
has allergies to nuts and eggs.

Mr Stamp has expressed a desire to lose weight and he has been referred to the Heart
Foundation and a weight loss weight to support organization. Since he has no regular GP, it
would be greatly appreciated if you see him and provide weight assessment and possible
psychological support.
