Dear Dr. Banerjee,

Re: Ms. Eleanor Bennet DOB: 06/12/1975

I am writing to request your review of Ms. Bennet, a 45-year-old lawyer who is

concerned about possible side effects of her current cardiac medication.

On 08/02/2021, Ms. Bennet suffered a heart attack after a long-haul flight and
was admitted to the Emergency Department. She was given a balloon-
expandable stent applied via groin. A week later, she was discharged on
captopril 50 mg twice a day and atorvastatin 80 mg daily. In addition, she was
referred for cardiac rehabilitation sessions, but she has been non-attendant
since then.

Today, Ms. Bennet presented with complaints of dizziness, light-headedness,

headaches and episodes of diarrhoea, which are caused by captopril taking. She
requests captopril discontinuation and is reluctant to start new medications.

Please note, Ms. Bennet drinks 40 units of alcohol weekly and smokes 20
cigarettes a day. She is overweight with a BMI of 29 and does little exercise. Her
father died of heart disease at the age of 53.

Based on the above information, I would like to request your review of Ms.
Bennet’s medication and advice on lifestyle changes. If you have any queries,
please feel free to contact me.

Yours sincerely,
Doctor
