Dear Supervisor,

I am writing to transfer back Mrs. Margaret Maggie Goldstein, who was admitted on
November 8 due to a right elbow dislocation. Your continuity of care is being requested.
INTRO 24

Mrs Goldstein presented to hospital after a fall in your nursing home. Subsequently, it was
noted that the affected area was healing slowly, although it showed to be intact; therefore,
a cuff with sling was applied. In addition, she developed urine incontinency on November 10
for which she was hesitant to use adult pads. BP1 53

During hospitalization, Mrs Goldstein was commenced on oxycodone 5mg, three times a
day when necessary, Atacand Plus 32/12.5 25 mg per day, and Remeron 15mg daily.
Regarding her mobility, she is able to mobilize with the use of a wheelchair while assistance
for transfers is needed. 45

Given the above information, Mrs Goldstein is medically stable and is ready to be discharged
back to your institution. It would be greatly appreciated if you could monitor the healing
progress of her elbow, provide incontinence care, and assisted showers. Please be reminded
that a physiotherapist appointment has been scheduled on November 28, 2017, 2pm at
Eventide. Lastly, her husband, Mr Solomon Goldstein, will escort her back to your facility.
