Dear Sir/Madam,

Re: Mrs Margaret Goldstein, 61-year-old

The purpose of this letter is to discharge back Mrs Margaret Goldstein to your care after
being admitted in our hospital on November 8, 2017 due to a dislocated right elbow. She will
be discharge on November 12, 2017 and your continuity of care is required.

Mrs Goldstein was presented to our hospital after acquiring a dislocated elbow due to a fall
incident at your nursing home. Whilst at the hospital, it was noted that her injured elbow
was healing slowly, hence, an elbow cuff with a sling was applied. In addition, it was
observed that he developed urinary incontinence which was managed with use of adult
diaper. Her medication include: Oxycodone 5mg 3/day to be continued as needed, Atacand
Plus 32/12.5 25mg/day and Remeron 15mg/day.

We are glad to inform you that Mrs. Goldstein is able to mobilise and transfer with the use of
a wheelchair and minimal assistance. Also, even though she is experiencing urine
incontinency, good skin integrity was well-maintained but please take note that she is
reluctant to wear disposable underwears.

As Mrs Goldstien is haemodynamically stable, we would like to request you to provide


continuous care to her following discharge by providing assistance during showers and
incontinent care. Equally important is to monitor her elbow’s healing process. Kindly be
reminded that a physiotherapist will be visiting her on November 28, 2017 at 2 PM in your
facility for follow-up. Lastly, her husband, Mr Goldstein will escort her to your facility.
