Re: Mas Maeve Greenson. D.D.B: 9 October 1958

Dear, Sir or Madam,

I am writing to introduce Mrs. Maeve Greenson, who was diagnosed with cancer of the lower intestine. He
requires a palliative case from your facility. Therefore, this gentleman has been transferred via ambulance with
her brother Mr. Brain. Hewson.

On March 2009, Mrs. Greens underwent partial bowel resection and a colostomy was performed
as it was found to be malignant. Following month radiation therapy was given for 6 weeks. She is widowed
with no children.

On 24 July 2009, Mrs. Greenson was brought to the hospital as he collapsed in his home with complaints of
dehydration, nausea, and severe pain, which was treated with Iv fluids. The following day nausea was
reduced.
Pain-free in intervals due to transdermal patch and analgesics were increased, However, severe edema
of lower extremities as well as bladder incontinence was observed.

On today's assessment Mrs, Greenson was very weak as well as get disoriented at times, in addition, she can
tolerate fluids. After the social worker's visit End of life case was
advice with family and patient consent as he has only 3-4 months to live. Please do not hesitate to contact me
