Re: Mr Gerald Baker, 79 years old

Dear Ms Bruin,

I would like to inform you that Mr Baker will be returned to your nursing home for continuity
of care. He was a known case of left total hip replacement and is ready for discharge today.

Mr Baker current conditions for his post-surgery site require daily dressings. Strengthening
exercises for his range of motion were applied and he has good mobilization noted. He is
weight-bearing with the use of a wheelie-walker and walks the length of the ward without
difficulty. Post-operatively he was disoriented to time and place possibly due to the
anaesthetic effect. His Hemoglobin is stable, but he needs to be monitored for anaemia. He
was also recommenced on 100mg of Aspirin every morning. He is also taking 2 Panadeine
Forte (co-codamol) four times a day as needed.

For his discharge plan, please monitor his Panadeine Forte (co-codamol) intake. Kindly,
observe his skin integrity and continue his exercise therapy. Take note that he requires a
toilet raiser. Lastly, he has a follow-up FBE and UEC test in City Hospital and staples removal
on 21 Septemeber.

Re: Mr Lionel Ramamurthy, a 63-year-old

Dear Ms Ponsford,

I am writing this discharge letter to inform you that Mr Ramamurthy will be discharged
tomorrow and he is returning back to your facility for continuity of care. He was a known
case of pneumonia admitted on 04 February 2019 at Newtown Public Hospital.

Mr. Ramamurthy had an acute shortness of breath, inspiratory and expiratory wheezing, as
well as a persistent cough noted which could trigger his chest and abdominal pain. He was
mobilizing with a pick-up frame and assisted with his daily living activities.

For his current condition, Mr Ramamurthy is afebrile and inflammatory markers are back to
normal. He is slowly walking, showering and toileting independently. He has still a dry
cough ,chest and abdominal pain. He is gaining weight of 1.5 kilograms as reviewed by the
dietitian. His overall health is progressing well and is ambulant.

I would be grateful if you could monitor Mr Ramamurthy’s overall health.


Please encourage to increase his oral dehydration and nutritious food intake like eggs, fruits
and vegetables. Routinely, please encourage also to do chest physiotherapy with deep
breathing and coughing exercises. Kindly ensure postural drainage by having him sitting
position. For chest and abdominal pain Paracetamol as needed and keep him warm all the
time.
