Dear Mr Reibert
Re Samuel Caine, aged 22 years old

I am writing to refer Mr Caine who was diagnosed with Hepatitis B. He requires further
evaluation of his condition and is now ready for discharge today.

Upon admission, Mr Caine presented fatiguability, nausea, poor appetite, belly pain, mild fever,
and jaundice. Liver Ultrasound and Hepatitis B panel were done. And, he mentioned that he has
a history of Hepatitis A seven years ago.

His current medication are Baraclude 0.5mg and Vitamin E 300mg. Both will be taken for 3
months. Also, Paracetamol and Ibuprofen 500 mg were prescribed for severe stomach pains
and high fever if necessary.

Apart from belly pain that he still experiences one in a while, Mr Caine has progressed well. His
appetite has improved, jaundice and occurrence of fever have lessened, nausea and fatigue
have alleviated. However, following his discharge, it is important to include fruits, vegetables,
high protein foods in his diet. Desserts and alcohol must be avoided. As well as, Exercise for at
least 30 minutes thrice a week is recommended.

I would be grateful if you could further monitor his condition. All the relevant documents are
