Re: Mrs. Jane LaPaglia, 71 years old

Dear Nurse,

I am referring to your care Mrs. LaPaglia who is diagnosed with Renal Failure secondary to
dehydration, mild dementia and pneumonia. She needs full time continuity of care following
her discharge from our facility today.

On 4 March, she was initially admitted in the Intensive Care Unit of our hospital in a coma state.
She was hooked into a mechanical ventilator for a week. In addition, Mrs. LaPaglia was ordered
to have dialysis to resolve her elevated creatinine and urea. During her stay, she acquired
pneumonia and experienced confusion after being transferred out from the ICU.
All of these were successfully treated and settled.

Mrs. LaPaglia and her husband cannot manage their personal needs as evidenced by their poor
hygiene and nutrition. Their son is a mechanic and only visits them on Tuesdays and Sundays.
Due to these reasons, the family decided to transfer Mrs. LaPaglia in your institution while her
husband relocates on a nearby hostel

It would be greatly appreciated if you could continue her oxygen therapy and execute UEC
blood test weekly. Please also ensure adequate hydration to retain renal function. Also,
reminding her to accomplish ADLs would also be beneficial.
