On this time and date RENIETO MACABUHAY REJANO 36 years old,
married resident of Brgy 2 San Juan Alaminos, Laguna personally appeared
before this office and reported:

That, he is a holder of PDL no. D09-98-071011 issued at San Pablo city

Land transportation Office.

That, on July 19, 2013 at San Pablo City he was apprehended for Viol of
Obstruction issued with TOP ( temporary operators permit no.
00000013577045.

That, said TOP ( temporary operators permit ) was lost in his position
and no to avail despite efforts and inquiries to locate the same.

Reportee appeared to this office presented his Affidavit of lost with Notary
Public signed by Alfredo Lao Belen Jr. CPA dated May 2, 2016.

National Crime Prevention Week

September 1, 2017 to September 7, 2017

Every first week of September of every year is declared as the

National Crime Prevention Week pursuant to Presidential
Proclamation No. 461 dated August 31, 1994. All government
agencies and instrumentalities of government are enjoined to
evolve and implement projects and activities that focus on
prevention as an approach in combating criminality.

This years theme is Pamayanang Mapagmasid, Kapayapaan

ang Maihahatid. This will focus on information drive and
awareness on the vital role of every stakeholder in crime
prevention through building collaborative partnership among the
community, law enforcement agencies and local executives that
gears to the full implementation of the Community and Service-
Oriented Policing (CSOP) System.
