Re: Mr Robert England, DOB: 09.11.1957

I am writing to request a weekly home visit for Mr Robert English, a -62-year old man who
was admitted on 19.05.2019 and underwent stent graft surgery. He is being discharged but
requires ongoing assessment and management.

During hospitalization, Mr England has made steady progress, and his vital signs are within
normal limits. His wound has been drained with a Resivac, and a Mepore dressing was been
applied to the surgical site, which has been sutured with dissolvable sutures. Additionally, IV
antibiotics have been given. He would benefit from observing regarding the risk of DVT as
well as checking the breathing since he is in upright position. It would be appreciated if you
could provide assistance with cleaning and dressing change for his wound.

In terms of Mr England’s lifestyle, he has been educated about reducing the alcohol and
unhealthy food intake. He has been suggested to increase dietary fibre. It would be
beneficial for him to be referred to a dietician to achieve and maintain an optimal weight. In
addition, he has been advised avoiding weight-bearing and strenuous exercise for 6 weeks
as well as gradually increasing gentle exercise. He is not permitted to drive for the next 4
weeks.

Regarding Mr England’s medical history, he has a history of hypertension for which he takes
Verapamil 80mg three time a day. a Low- dosage aspirin has been prescribed after
discharge.

Please note, a review has been scheduled on 31.05.2019 in Outpatient Department.
