Dear Mr Pansford,

Re: Lionel Ramomurthy, aged 63

Thank you for continuing the care to widowed Mr Lionel Ramamurthy, who has been suffering from
pneumonia is widowed. She is ready to be discharged/for discharge back (in)to your retirement
house.

She was admitted on 4th February with pneumonia following by shortness of breath, persistent
cough, fever and loss of her weight.

İn addition Initially, she had a weight loss of about 4 kg in the last 6 months due to/because of be
pooor her poor diet, so she a special diet was arranged for her a special diet by her dietitian(place of
the info??? To the very end?) She was suffering from fever, tremor and sleeplessness. These are
problems was taken under control. Beside this she has oteoarthritis and decrease of eyesight
menaged diclofenac medication.

She wil be need a asistans for her mobilizing with her frame. She is also need activities of dailiy living
such as showering , dressing etc.
