Scenario 1.

You are a Community Health Nurse in Brgy. Masigla and went for home visit for Mabunga
Family. Mr. Rey, the head of the family, is a farmer and his wife Felly is a “labandera”. They have 3
children named Pia who is 10 years old, Rj a 7 years old and Tina a 4 years old.
When you reached their home, Mrs. Felly was cleaning their house while Pia was taking care of
their youngest sibling. Mrs. Felly entertained you and you let her rest then took her Blood Pressure. Her
Blood pressure was 160/100mmHg. After 30 minutes you took her BP again resulting in 150/100mmHg,
you decided to retake it after 30 minutes and her BP result was still 150/100mmHg. You educate her
some home management on how to decrease her blood pressure and advised her to visit the nearest
Health Center. All the data was recorded and made an appointment visit on the next day.
On the following day, Mrs. Felly told you that she was able to visit the Health Center yesterday
and she was given Losartan (Bepsar) 50mg once a day to be taken at 7am and an emergency drug which
is Metropolol (Lopressor) 50mg PRN sublingually.

Tasks:

1. Discuss briefly why Losartan (Bepsar) and Metropolol (Lopressor) weres given to Fely.
2. List at least two (2) health education topics you can discuss with Fely and her family.
3. Formulate two (2) priority family nursing care plans (FNCP) based on the scenario.

Scenario 2.

You are taking care of the Dimalanta Family, Mr. Jose is a tricycle driver and his wife Rina is a
full time housewife, they have 2 children named Ana who is 8 years old and Bruno who is 4 years old.
While doing your home visit you noticed that their wooden stair case was broken and there’s a
pointed part of the wood that was lifted. Along the side you saw that the Zonrox Bleach, Oxalic Acid
that Mrs. Rina usually uses for laundry and Muriatic Acid for cleaning their Comfort Room were
improperly kept. You educate Mrs. Rina on what possible health risks are in this situation.
All the data and observation was recorded and made an appointment visit.

Tasks:
1. What are your initial actions as Community Health Nurse that to this family?
2. Identify the age group of Ana and Bruno and their stages of development according to the Cognitive
Theory.
3. List two (2) health education topics that you are going to discuss with the Dimalanta Family.
4. Formulate at least two (2) priority family nursing care plans (FNCP) according to the scenario.

Scenario 3.
Mr. Chris is a Security Guard while his wife Susan is a 30 years old Fruit Vendor have been
married for two years. They are currently residing at their own house. Mrs. Susan noticed that she’s having a
delayed menstruation for 3 weeks; her last menstrual period was October 7, 2019. She immediately bought a
pregnancy test and the result was positive. She reported it to her husband and the husband said “Am I ready to
be a father?”
The married couple visited the Health Center and they are advised to have at least 4x maternal check-
ups during her pregnancy. On her 4th month of pregnancy she was given Iron 60mg/tab OD, Folic Acid 400µg
(0.4mg)/tab OD and Vitamin A 10,000 IU to be taken 2 times a week for 4 weeks.
You made a home visit and Mrs. Susan is now on her 6th month of pregnancy. You took her Vital Signs:
BP: 110/70 mmHg, Temp: 36.7°C, HR: 103 bpm, RR: 19 cpm. After getting her vital signs, she asks you what
are the possible home management for backache due to pregnancy. She also happily said they will be having a
baby boy.

Tasks:

1. Compute the Expected Date of Delivery/Confinement (EDD/EDC)


2. Discuss briefly why Iron, Folic acid and Vitamin A were given to Susan.
3. List at least three (3) health education that you should give to Mr. Chris and Mrs. Susan.
4. Identify what level of assessment should you use in this case.
5. Formulate two (2) priority family nursing care plans in consideration that they’re having their first baby.
