NARRATIVE REPORT
HOME VISITATION
October 18, 2023

On October 18th, Ms. Magbanua conducted a home visitation for Lucky Guadayo, prompted by the
unresponsiveness of his mom to phone calls and her failure to attend a requested visit to the school. The
objective was to address concerns regarding Lucky's consistent absenteeism despite a prior agreement made on
September 25th. This visit aimed to understand the challenges contributing to Lucky's attendance issues.

During the home visitation, it became evident that, despite the earlier agreement, Lucky continued to
face challenges attending his classes regularly. Recognizing the persistence of this issue, a new agreement was
discussed and signed during the visit. Mrs. Guadayo acknowledged the difficulty in motivating Lucky to attend
his classes and shared additional insights into the challenges they were facing in their living situation.

In light of these challenges, the school, Lucky, and Mrs. Guadayo collaboratively signed a renewed
agreement, outlining specific measures to address and overcome the barriers hindering Lucky's attendance. This
agreement not only reinforced the commitment made during the previous visit but also acknowledged the
ongoing difficulties and sought to find sustainable solutions.

Moving forward, the teacher will actively support and monitor Lucky's attendance, working closely with
Mrs. Guadayo to provide the necessary assistance. The collaborative effort between the school and the family is
crucial in ensuring a conducive learning environment for Lucky, and the signed agreement serves as a testament
to their shared commitment to his academic success. The teacher appreciates Mrs. Guadayo's openness during
the home visitation and is dedicated to providing continued support for Lucky's educational journey.
