Thank you for applying to the New Jersey Board of Nursing for Registered Professional Nurse Candidate licensure.
In order for the Board to continue processing your application, the documents listed below must be uploaded to your
application file. Please log in to your MyLicense Online account and upload the necessary documents which will
facilitate your application process. If you choose to have a single state license; please write a letter stating: I want
to be issued a single state license and upload your letter. If you previously uploaded a particular document online, it
may not have been accepted by the Board, so please re-upload the document if requested.

 Please upload proof of NJ residency, i.e., NJ driver's license or NJ Motor Vehicle ID.
 Your Criminal History Background Check has not been completed. Please be advised that New Jersey law
(N.J.S.A. 45:1-28 et seq.) requires all applicants to undergo a criminal history background check as a
condition for licensure. Instructions to complete your Criminal History Background Check process have or
will be emailed to you separately (i.e. initial fingerprinting or resubmittal if you have been fingerprinted for
the Division previously).

If you need testing accommodations, please submit a letter from the treating physician with a diagnosis and code.
Letter from the school stating what accommodations were offered in the program, and a letter from detailing what
accommodations you require.

Please note the applications are valid for one year. If you have not submitted any new information to your
application, the application will be deleted, and you will have to submit another application along with an
application fee.

You can check the status of your application by logging-in to your MyLicense Online account. If you have
additional questions or concerns regarding your application, please contact the Board office at 973-504-6430,
between the hours of 8:30 a.m. and 5:00 p.m., Monday through Friday.
