910926016130 was confirmed positive for covid-19 on 18 March 202. Accordingly, I
would like to make an application for a claim for social benefits that have been provided
for former covid-19 patients.

17/03/2022 I worked as usual in-patient ward at Maria Hospital from 7am -2pm.
During working hours, I feel a bit uncomfortable as I started to have sore throat and
headache. After working hours, I suspected I might be Covid positive and continued to do
Covid 19 self -screening tests (RTK-Nose Swab) because there were some Maria hospital
staff was already quarantined due Covid was positive. The result was negative.

My symtoms become worse as I started having fever ,headache ,dizziness , joint pain ,
body aches and sore throat.I did a second Covid 19 self -screening tests (RTK-Nose Swab).
The result was positive for Covid 19. I immediately rushed to the nearest clinic (JJ
Clinic )to RTK test . The result from was Covid positive .

I straight away notified my hospital that I tested Covid 19 Positive .
