The extraordinary situation may have changed our ways, but as Industrial Engineers we shall adapt to new approaches. Thus, leading us to this partnership proposal of offering Online Lean Six Sigma Yellow Belt Certification to a target of 150 Industrial Engineering Students all over the Philippines in May 2021.#IExpand: Lean Six Sigma Belt Certification Registration Guidelines: LSSYB Certification is open to all students nationwide regardless of their program. Registration period starts from March 17, 2021 and ends on April 28, 2021. All participants shall accomplish the registration form within the registration period Registration will be facilitated through Google Forms; participants must go bit.ly/IExpandRegistration to access the registration form. Slots for the LSSYB Certification will only be reserved if the participant has paid the reservation fee amounting to PHP 1,500. In order to take the certification exam, participants should be fully paid Note: Necessary requirements below shall be compiled on one document each following the file and name format and attached in the link provided. This code will be used by the organizers to identify which group you belong and to verify if you qualify for the group discount. Note: The representative should be the first to accomplish the registration form. 5. Upon registration, the representative will create a unique 6-digit code that will be used by the group members when they register (the pin number will indicate that you belong to the same group). 6. PAYMENT GUIDELINES: Promo: Each participant should be paid of at least PHP 1,5000 before the event proper for the reservation fee. Each participant must pay the following fees: Student (PIE Member) - PHP 2,800 Student (Non-PIIE Member) _- PHP 2,900 There is a discount for the PIIE Member student if they register as a group of § person. Payment shall be made to PIIE-National Student Chapter bank account. Note: Payment details will be disclose in your registration confirmation After you make the transaction, please take a screenshot/picture of the receipt of payment transfer. Once the pament is done, it is non-refundable. Wait for the email of Finance Committee to confirm your payment. The person whose name is registered should be the one to attend the event. No slot replacement is allowed.
